For parents and guardians of child artists, please be advised that we are requesting information pertaining to your child and that you should be aware that this information is being solely collected for review by the SMA. By filling out this Interest Application form and pressing -Send- button, you are providing parental consent to send your childs information to SMA. As the parent or guardian, I understand that transportation is my responsibility and that I will be notified of any problems involving my childs SMA involvement. Please be advised that you will be required to fill out an SMA Parental Release Form upon your childs acceptance to the SMA.
